Red zone offense
The 49ers have one of the league's best running games, yet look at their points per game. With a team that can run as well as the 49ers, that means their problem isn't moving the ball. Between the 20's, the 49ers are one of the best offenses in the NFL, but inside the 20's they become one of the worst. And if Nedney misses a kick or gets blocked, that pretty much nullifies all those Frank Gore runs you see on NFLN, doesn't it? Don't worry about the players, the 49ers have the players to make it work. Now they just need to execute. There is no reason why their running game and a tight end like Vernon Davis, combined with Alex Smith, can't be a VERY effective offense inside the 20. And yet they aren't. That needs to be a major point of emphasis in the offseason.
Defensive personnel
This is just the opposite of the offense. While the offense has the talent and not the execution, the defense has actually executed pretty well, but they just lack the talent to really become a great defense. The fact the defense has looked as good as it has, with the players they are trotting out there, is phenomenal. But eventually good teams will expose weak players like Sammy Davis and the myriad of other rejects in our secondary. Some of it is youth (as is the case for Keith Lewis, Marcus Hudson et al), but most of it is just plain sorry-*** players. If the team can upgrade the PLAYERS by next season, and keep the same discipline they have this year, they'll be very difficult.
People might be surprised to hear this, but I feel that if the 49ers improve these 2 areas, there is no limit to what the team can do next year. None. But keep in mind these are 2 problems that don't get solved overnight. Scoring inside the 20's is the hardest thing to do because the defense has less area to defend. And getting the right players in the secondary is hard too, especially when you're the 49ers and you don't have the sufficient luxury box revenue to compete in the free agent market.
